Documentation Form

Unusual Bird Sightings in Iowa

A documentation form is requested by the Iowa Records Committee and Seasonal Field Reports Editors for all casual and accidental sightings as well as extraordinarily early and late dates.  Field reports editors may also request documentation for new populations of regular but not widespread species.  Use the following links as guides for documentations: Iowa Review Species and Early-Late Dates.  Be sure to keep a list of other interesting observations and report to the Seasonal Field Reports.

Not sure how to write a documentation or what constitutes a report that may successfully go through the committee review process?  If you are unsure, please read the helpful articles linked from our Records Commitee main page.

We appreciate your submitting your report through this on-line form.  A printable copy for your records is available when you finish.  Because the application may "time out" before you are finished writing, some folks find it easier to type up their details first and then copy and paste when they are ready.  The Seasonal Field Reports Editor and Records Committee Secretary will be notified immediately of on-line submissions for use in their reports and the data will be available on-line for future researchers.  If you are logged in, your form will be pre-populated with your name and address and you will be able to retrieve any documentations you have submitted.
